2018-2019 Special Events	A brief calendar of our school's special events
Application Form	Submit this form to the office to apply for our day care. All information must be completed, especially date of birth
Authorized Escorts List Form	Parent can authorize other individuals other than parents to pick up child. If parents are the only people picking child up, then parent can just fill out the top and sign the bottom of the form
CACFP Form	The center gets federal funds for meals served based on this form's information
Health Examination Form	Must be completed by child's physician prior to first day of class
Cumulative Health Form	A record of child's medical and health history
Parent's Handbook	Important day care policies and information. Parents must sign each page
Student Emergency Contact & Pick up form	Indicates person(s) to contact in case of emergency and who is allowed/not allowed to pick up child
